PreRegistrationIf you wish to register please attend the surgery to collect the registration forms. You will need to fill a form in for each person registering with the practice and provide identity documents. At least one should be a photo ID and you will also require proof of address.

The documents you could use for ID include:

  • Passport
  • Driving License
  • National ID card
  • Bus Pass
  • Rental agreement
  • Mortgage agreement
  • Utility Bill
  • Bank statement

If you do not have the required identity documents please ask at reception for further advice on how you can register with us.

You must also obtain your NHS number from your previous practice if you have been registered at a GP practice elsewhere.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

Please note that our registration clerk processes new registration requests on one day each week and due to the large volume of requests we receive there may be a few days delay before we can get you onto our system. 

Please note that we no longer process requests to be added to the National Donor Register so please do not complete this part of the form. If you wish to join the register please contact them direct on
